This form should be filled out by guests after the sponsor has submitted the guest nomination form to the Membership Vice President. This should be filled out in addition to normal registration form.

To help us understand your quartet singing experience, please complete only the applicable parts of the following questionnaire. Participation in the brigade weekend is not dependent upon on your answers; however your answers will help us understand your skills and ability to play an equal role with the rest of the singers in attendance.
1. List up to your last three Quartets. Specify if the quartet(s) were registered, List if, where and when your quartet(s) competed for scores. What was your highest quartet score? List your voice part(s), other members, or distinctions earned (contest or otherwise) and any other information that would help us gauge the likelihood for you to enjoy and contribute to the GLHB weekend:
